Sherman West Court opened in 1991 as a 120-bed independent and skilled nursing care facility. It is located at 1950 Larkin Ave. in Elgin, and now offers 74 rehabilitation beds and 38 skilled nursing beds, with private room options. Services include rehabilitation, long-term care, respite and hospice services, as well as short-term post-acute care. The facility sees nearly 1,000 admissions each year.

Advocate Sherman West Court is regularly recognized as one of the best in the nation. West Court has received the highest possible overall rating of 5 stars from US News & World Report’s annual “Best Nursing Homes Report”, multiple years in a row.  In August 2016, West Court once again received a 5-star rating from Centers for Medicare & Medicaid Services (CMS), for achieving excellence in Quality Measures, Staffing, and Health Inspections

Full Time. Rotating weekends and holidays

Days 7a to 3:30pm 

The RN assumes primary responsibility for the assessment, planning, implementation, and evaluation of nursing plan of care for participants at the Skilled Nursing Facility.

This individual demonstrates clinical knowledge at the unit level and begins to apply this knowledge to a specific patient population. Positively influences practice and serves as a role model in the clinical setting. Anticipates changes in patient situations and intervenes appropriately.

Nurse/Patient Relationship: Develops and maintains a therapeutic relationship throughout the healthcare continuum
Assuring/Improving Quality Care: Facilitates the patient and family's right to receive quality, cost effective care
Client/Staff Education: Demonstrates a commitment to meeting the learning needs of patients and families. Utilizes appropriate resources to meet those needs and achieve positive patient outcomes. Assists in creating an environment that promotes educational growth opportunities for nursing peers and colleagues.
Nursing Process: Demonstrates the use of nursing process as a problem~solving model.
Leadership/Collaboration/Professional Development: Demonstrates respect and understanding for peers and other clinical disciplines and participates as an effective member of the patient care team to formulate an integrated approach to care. Demonstrates leadership behaviors and is committed to the development of self and others.

Required Qualifications:

Graduate from an accredited school of nursing
Good time management and organizational skills
Current Illinois RN license
Current CPR
Ability to transport objects, patients, and equipment
Ability to respond quickly to patient needs and work at fast pace
Work environment includes probability of exposure to adverse, hazardous, or unpleasant conditions while caring for the sick
Ability to assist in evacuation of patients in the event of a fire or other disaster
Routinely works with potentially infectious specimens and patients
Ability to respond to patients in a timely manner and access all areas of the unit
Ability to lift up to 100 pounds occasionally and/or in excess of 50 pounds frequently and/or in excess of 20 pounds constantly
If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.
