Job Description
To oversee the efficient management of the OPCM care managers and related operations of the OPCM program. Oversees outpatient care managers who coordinate care, educate members on disease management, facilitate communication between medical facilities and physicians via relationships with members/patients/families, hospital staff , home health agencies and community~based agencies. Ensures that members are provided with quality service in accordance with benefit coverage guidelines and department and regulatory agency standards with cost effective results. Works with medical/social work professionals to meet department goals while demonstrating excellent measures of clinical quality and patient safety.

Accountabilities:

  1. Supervision of Outpatient Care Managers to assure an efficient Outpatient Care Management program promoting quality and comprehensive care for HMO members and attributed patients. Promotes effective outpatient care management techniques in a complex environment that encompasses high risk,pharmacy costs, transplants and complex illnesses within a medically and socio~economically diverse patient population
  2. Supervises staff's compliance with department standards, policies and procedures, as well as regulatory agencies and contractual requirements.
  3. Participates in daily OPCM activities in accordance with department standards in order to facilitate outpatient care management, promote comprehensive care, improve health outcomes and overcome barriers to health. Oversees and act as a resource to outpatient care managers who are serving HMO and attributed patients in an outpatient setting. Supports care managers to ensure that Advocate and MCO/MC policies are followed including documentation standards and benefit issues
  4. Communication with internal and external customers in a positive and professional fashion to promote member satisfaction, quality of care delivered and the coordination and integration of multi~department/system activities.
