What You Will Do:
The HCC Clinical Documentation Improvement Chart Auditor is responsible for functioning as a clinical documentation specialist. The position responsibilities include, but are not limited to the review of paper and electronic health records to validate complete documentation of all relevant diagnoses, quality assurance efforts, and education for all risk insurance plans.

Accountabilities:

  • Responsible for chart review
  • Responsible for claims review
  • Responsible to assist in the development of coding educational plans, materials, and resources to educate physicians and associates with respect to the overall objective of the APP risk adjustment coding program.
  • Responsible for maintaining industry knowledge

 

What You Will Need:

  • Bachelor's Degree in Health Information Management plus 3 years related experience in physician coding OR Associate's Degree in related field plus 5 years related experience in physician coding.
  • RHIT, RHIA or CDIP
  • Previous auditing/chart reviews experience.
  • Ability to interpret documents such as, but not limited to encounter forms, medical records, physician documentation, lab reports, dictated reports, operating instructions, and policy/procedure manuals.
  • Knowledge of medical and insurance reimbursement, ICD9, ICD10 and CPT coding, disease processes and medical terminology.
  • Ability to interview people to obtain basic information.
  • Ability to write correspondence proficiency.
  • Ability to interact with all levels of health care team professionally.
  • Ability to deal with problems involving several abstract and concrete variables in standardized situations.
  • Ability to meet deadlines and goals.
  • Effective and professional communications skills high volume and multiple tasks.
  • RHIT, RHIA or CDIP
  • Proficient in PC skills include Microsoft Word and Excel.
  • Ability to drive or commute to various sites to gather records for chart review or attend meetings.
  • Flexible hours to meet the needs of the practice
  • Ability to communicate in a professional manner and effectively handles difficult situations and/or individuals objectively.
  • Ability to be self~directed and meet goals with minimal supervision
  • Demonstrates the ability to train/educate others as needed
