Position Requirements:
AA degree
3~4 years experience working in a managed care organization, hospital or physicians office.



Good communication skills. Ability to work as a team member. Competent computer keyboard, laptop manuvering, competent in Windows, Excel, database entry.
Good organizational skills. Ability to handle difficult calls and maintain professional conduct.
Willingness to be flexible with job responsibilities

Competent Medical terminology ability/experience

Ability to sit for prolonged periods of time as this is an office based position
Ability to manage multiple systems at one time
Ability to work under stressful conditions.
Excellent phone mannerisms, ability to effectively engage conversation, motivate and engage patients to participate in our program, and speak to all types of patients, particularly seniors.
Regional travel required for meetings
May need to flex daytime hours to stretch to 8pm M~F , and Saturday mornings to reach pts after work hours.




Purpose:
The Clinical Operations Assistant (COA) role is to assist in key workflow tasks to enhance the efficiency of the Care Transitions Team/Care Management Clinicians. This can include database and software searches, entry, report generation and distribution, patient eligibility verification, coordination of services and medical management activities, including patient outreach, enrollment and assessment, gathering care management information and correspondence with patients in order to facilitate quality and timely services provided to our patients and physicians.

Accountabilities:
Data Collection and Reporting
Effectively and Efficiently communicates with patients and healthcare professionals to ensure smooth care transitions.
Assist in the care management/CTT process
