The purpose of this position is to help achieve the Vision of Advocate Aurora Home Health Services by performing the duties of this position in a cost effective, value- based delivery system in a results -oriented and customer focused manner. This position is a critical component of the operating unit and as such meeting individual performance expectations and contribution to team performance goals is required.

Provides managerial direction to ensure delivery of quality- cost effective and timely services to patients and families. Ensures and maintains patient care delivery in compliance with all local- state and federal regulations- accreditation standards and department objectives. Contributes to the strategic direction of Home Care by providing planning and development of staff and patient care initiatives. Assures adequate exchange of information and communication among associates. Coordinates the implementation of the interdisciplinary plan of care.


  • Bachelors degree in Nursing Physical Therapy, Doctorate Physical Therapy or other healthcare professional degree
  • 1-2 years experience in a Medicare licensed home healthcare agency
  • 2-3 years supervisory experience
  • Excellent organizational, communication, and interpersonal skills.
  • Current licensure in State of Illinois in your field of discipline.
  • CPR certified.
  • Ability to travel between Advocate Health Care branche offices and to client meetings in all types of weather on all types of roads.
