Master's Degree (MSN, MS, etc.) required
5+ years recent experience in clinical specialty, transplant experience preferred
Graduate from an accredited school of nursing Strong analytical skills
Proven leadership skills
Good organizational skills
Basic computer knowledge
Effective communication and presentation skills. Current (or eligible) advance practice RN license in the State of Illinois
Certification in specialty
CPR certification.
This position will need to recognize the needs and behaviors of a variety of age groups of patients treated i.e., understand child growth and development, normal patterns of adolescent behavior and look for signs of normal aging prior experience with various age groups is preferred.
Ability to work flexible hours according to customer needs.
May be exposed to hazardous materials and life threatening diseases.
Ability to prioritize independently and respond to multiple simultaneous requests.
Lifting to 20 pounds occasionally and /or up to 10 pounds frequently.
Ability to communicate with patients/families
Ability to travel locally and nationally.
Ability to work within stressful conditions and difficult situations. If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.

Provides a high degree of professional competence and demonstrates a high level of autonomy in the care of heart transplant patients, heart failure patients and cardiovascular nursing. The scope of practice will impact patient care through direct patient contact, staff development and/or policy development based on sound standards of care. Advanced practice nursing has evolved into the roles of the clinical nurse specialist. CNS's integrate education, research, management, leadership, and consultation into clinical roles, and they function in collegial relationships with nursing peers and other professionals and individuals that influence the health environment.
Heart Transplant Coordinators are highly developed clinical nurse specialists who independently (under a collaborative practice agreement with transplant physicians and surgeons) monitor and manage a case load of Heart Transplant patients 24~hours a day. They provide clinical, educational, and consultative leadership and support to the patients and their families, the cardiac surgery team, nursing staff and ancillary heath care associates.

Accountabilities:
Practitioner: Demonstrates expert clinical judgment and autonomy, provides health care providers with recommended guidelines to achieve optimal level of practice.
Administrative: Demonstrates leadership qualities that reflect accountability and interdependence consistent with the goals of the organization.
Research: Promotes research findings by making appropriate recommendations for change in policies, procedures and practice
Educator: Develops and provides ongoing education programs for health care providers to ensure availability of current information regarding patient care and maintenance/enhancement of clinical skills.
Consultant: Serves as a consultant and/or resource person for hospital personnel in area of specialization. Accepts referrals from all units of the hospital and collaborates with physicians, nurses and other members of the interdisciplinary team in planning the care of patients needing specialized nursing. Initiates referral to other healthcare providers and resource agencies as necessary
