Rehab Clinical Coordinator, Advocate At Home, Lombard, Illinois 

  • Full-time,  (Hours generally 8:00-4:30 Monday-Friday 40 hrs/wk.)

  • The Rehabilitation Coordinator will be responsible for the clinical coordination of rehabilitation services on identified patient populations, and support individual clinician's accountability for achieving the organization's objectives.
  • Additionally, this position will promote high quality, safe, and cost-effective care by assisting in the creation and supervision of clinical protocols, development and presentation of educational programing, help coordinate and perform CQI projects, and create and sustain an environment that demonstrates a commitment to customer satisfaction, collaborative care, and communication to both internal and external customers and providers of service.
  • The Rehabilitation Coordinator will coordinate and supervise student programs, assist in staff training and orientation, and support clinical competency programs.


Position Requirements:

  • Illinois Licensure in Physical Therapy
  • CPR Certification
  • Entry level degree in Physical Therapy or higher
  • Evidence of participation in course work in Education, Business, Leadership OR evidence of at least 1 yr leadership experience OR course work for APTA Clinical specialist certification and evidence of clinical leadership experience.
  • 2 years recent experience in home care physical therapy in the US, and 5 total years of full time clinical experience
  • Knowledge of federal and state homecare regulations, and IL practice acts of rehabilitation services.
  • Member of APTA
  • Strong interpersonal skills and communication.
  • Strong ability to convey / teach information to individuals
  • Sound analytical and problem solving skills.
  • Sound organizational skills
  • Basic knowledge of computer skills including keyboarding, Word, Excel, and Powerpoint
  • Speaks and writes English clearly, intelligibly and fluently.
  • Lifts up to 20 pounds infrequently and/or up to 10 pounds frequently
  • Work requires ability to stand/walk with intermittent stairclimbing.
  • May be required to make home visits so must be functional mobile to infrequently position, transfer, or manually treat patients. Possible exposure to chemicals or blood~borne pathogens
  • Use of EMR and computer requires repetitive use of keyboard and mouse. ability to visually see monitor or displays.
  • May occasionally rotate through weekend oncall.
  • Special events and sponsorship of educational activities may require weekend and evening hours resulting in occasionally exceed 40 hours per week.
  • Ability to travel between branches and care treatment sites. Able to provide own transportation.
  • This requirement supersedes any previous lifting requirement effective 08/01/2014. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.
