REGISTERED NURSE-HOSPICE LIAISON

 

Location: Good Shepherd Hospital Barrington and Sherman Hospital in Elgin, IL and the surrounding skilled facilities

Shift: Day--position will be 11am-7:30pm and one weekend per mon.  No on call rotation.  One summer and one winter holiday will be required.

JOB and UNIT SUMMARY: 

 

To develop and implement sales plan and related tactics aimed at increasing hospice market share. Responsible for generating new business, maintaining current referral relationships and achieving revenue goals in accordance to division growth strategies. Accountable for presentation and consent of hospital patients. · Identifies prospective referral sources through needs and new opportunities, focusing on hospice services. Practices and creates an environment that supports and sustains effective communication with internal and external customers. Develops and sustains relationships that enhances effective coordination of care and increases referral base. Responsible for building and maintaining excellent customer relationships with Advocate Hospice key referral sources as identified by the Manager of Intake, Account Representative and Director.    · Promote continuity of care by facilitating appropriate and timely referrals of designated patients to Advocate Hospice. Upon referral to Advocate: 

 

 

REQUIRED QUALIFICATIONS: 

 

· Graduate from accredited school of nursing

· RN with 2~4 years experience working with physicians and in hospitals required, BSN preferred. Hospice experience recommended

· Comfortable working with patients and families at end of life stages

· Ability to function independently 

· Ability to travel to local SNFs in community

· Strong documentation, communication and computer skills
