After Hours Weekday RN-Hospice

Hours--T-F 5pm-8am; must be able to work one summer and one winter holiday, no on call or weekend requirement


  • Responsible for achievement of agency, department and personal goals and objectives.
  • Provides and performs patient visits as assigned and as needed for appropriate hospice care.
  • Responsible for providing visits and consultation to patients, families, and health care providers regarding hospice services and end~of~life treatment choices as well as performing the admission process.
  • Maintains clinical records in accordance with established policies and procedures, state and federal regulations and Accrediting Organization accreditation standards



  • Minimum of three years of clinical experience in a health care setting with emphasis on physical assessment, triage, and communication one year of hospice experience preferred
  • Current RN licensure in state of Illinois

  • If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.



  • The purpose of this position is to help achieve the vision of Advocate Hospice Services by performing the duties of this position in a cost effective, results oriented and customer focused manner.
  • This position is a critical component of the interdisciplinary team and as such meeting individual performance expectations and contributions to team performance goals is required. 
  • The after-hours nurse provides for continuity of care by coordination of care through triaging, patient visits and admissions.


This associate will work M-T-W-TH from 5pm-8am for triage, visits and admissions
