Position Requirements:
High School education
3~5 years prior experience in a medical field such as a hospital or medical office.
Good communication skills
Typing 25 wpm, preferred Experience using Xerox, fax, multi~line phones Exposure to PC use None This job requires reaching, bending, stooping, lifting. Ability to work in a fast~paced, changeable environment. Must be willing to work occasional overtime if needed.

If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.

Purpose:
The purpose of this position is to help achieve the vision of Advocate Home Health/Hospice Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the team structure within the Service Office and as such meeting individual performance and contribution to team performance goal is required.
The Hospice PSA position supports the intake and operational activities for Advocate Hospice. Responsibilities include coordinating visits (nursing, home health aide and physician), monitoring productivity of HHA staff and adjusting schedules as appropriate, maintenance of the data base of referrals and admissions, receive inbound calls and triage appropriately.

Accountabilities:
Maintains staff schedules and schedules patients for admission.
Responsible for support function and timeliness of execution to meet the required regulations.
Responsibility for the screening of referrals to ensure all new patients meet agency admission criteria and comply with state and federal regulations.
Ensures professional courtesy for effective triage/routing of phone calls and patient privacy.
