Position Requirements:

  • Must be able to work weekends or rotate to weekends
  • HS Diploma or equivalent.
  • 3 years of hospice intake experience
  • Very good communication skills.
  • Knowledge of Medicare rules and regulations.
  • Customer Service skills
  • Working knowledge of managed care environment
  • On call rotation.
  • Ability to work in a fast paced environment with established time constraints.


If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.

Purpose:
The purpose of this position is to help achieve the vision of Advocate at Home Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required. Responsible for overall coordination of hospice referrals from receipt of referral through completion of the admission process.


Accountabilities:
Responsibility for the coordination of all hospice referrals and comply with state and federal regulations. Serves as the internal point person and processing agent for all new hospice referrals.
Provision of Quality~ Maintains collaborative relationship with referral sources. Serves as the hospice point person for referral sources to coordinate the referral to admission process.
