Position Requirements:

  • Bachelors degree required (BA, BS, BSN, etc.)
  • Current RN licensure in Illinois
  • 3 years of relevant clinical experience
  • 3+ years of Care Management experience
  • Demonstrated leadership experience
  • Good customer orientation
  • Organization and prioritization skills
  • Planning and delegation skills
  • Basic team building skills
  • Utilization of critical thinking in timely decision making
  • Basic computer skills
  • Effective communication skills in a group setting
  • Ability to work within a rapid changing environment

  • 24-Hour accountability for Care Management issues

Purpose:
To oversee the human resource function and daily operations of the Care Management Department. Oversees the discharge planning, care management, social work, and government audit functions.

Accountabilities:

  • Assists in preparation and monitoring of the Care Management budget.
  • Manages the government audit process.
  • Maintains personal and professional growth and development and competence in the care management field.
  • Manages the care management and social work functions.
  • Manages the human resource function for the Care Management Department.
  • Oversees the discharge planning process for the hospital.
  • Oversees the utilization review function.
