Position Requirements:
RN, BSN BSPT, DPT or other healthcare professional degree
3 years medical/surgical experience
1~2years experience in a Medicare licensed agency preferred
1 year supervisory or equivalent experience
Excellent organizational, communication, and inter~personal skills.
Current licensure in State of Illinois in your field of discipline.
CPR certified.
Will participate in managerial on-call rotation
May be called upon to do field visits.
Good analytical skills.
Excellent interpersonal/communication skills.
Must be knowledgeable of regulatory home care guidelines and admission criteria.
Ability to travel between AHHS branches and to client meetings in all types of weather on all types of roads.
Required to be on~call (carry pager).

Purpose:
The purpose of this position is to help achieve the Vision of Advocate Home Health Services by performing the duties of this position in a cost effective, value~based delivery system in a results oriented and customer focused manner. This position is a critical component of the operating unit and as such meeting individual performance expectations and contribution to team performance goals is required.

Provides managerial direction to ensure delivery of quality~ cost effective and timely services to patients and families. Ensures and maintains patient care delivery in compliance with all local~ state and federal regulations~ accreditation standards and department objectives. Contributes to the strategic direction of Home Care by providing planning and development of staff and patient care initiatives. Assures adequate exchange of information and communication among associates. Coordinates the implementation of the interdisciplinary plan of care.

Other duties as assigned. examples include active participation in various quality, service or process improvement committees or workgroups.
Participates in achievement of KRAs in the areas of Reimbursement and Fiscal Management
Participates in achievement of Organizational and Branch Key Result Areas (KRAs) related to Associate Engagement, Patient Experience and Health Outcomes.
