Advocate at Home, the home care division of Advocate Aurora Health, provides home care, hospice care, home infusion, and home medical equipment/respiratory therapy for patients throughout Illinois and Wisconsin. Advocate Aurora at Home is part of Advocate’s Post Acute Division, which also includes the Post Acute Network (PAN)— select skilled nursing facilities, rehabilitation services providers, assisted living communities and long-term acute care hospitals that support Advocate’s efforts to ensure that the good care provided at our hospitals can continue following discharge.

Overview

The purpose of this position is to help achieve the Vision of Advocate Aurora Home Health Services by performing the duties of this position in a cost effective, value- based delivery system in a results -oriented and customer focused manner. This position is a critical component of the operating unit and as such meeting individual performance expectations and contribution to team performance goals is required.

Provides managerial direction to ensure delivery of quality~ cost effective and timely services to patients and families. Ensures and maintains patient care delivery in compliance with all local~ state and federal regulations- accreditation standards and department objectives. Contributes to the strategic direction of Home Care by providing planning and development of staff and patient care initiatives. Assures adequate exchange of information and communication among associates. Coordinates the implementation of the interdisciplinary plan of care.

 

Qualifications

  • RN, BSN BSPT, DPT or other healthcare professional degree
  • 1-2 years experience in a Medicare licensed agency preferred
  • 2-3 years supervisory or equivalent experience
  • Excellent organizational, communication, and interpersonal skills.
  • Current licensure in State of Illinois in your field of discipline.
  • CPR certified.
  • Must be knowledgeable of regulatory home care guidelines and admission criteria.
     
    Accountabilities:
  1. Staff and Patient Care Management
  2. Other duties as assigned. examples include active participation in various quality, service or process improvement committees or workgroups.
  3. Participates in achievement of KRAs in the areas of Reimbursement and Fiscal Management
  4. Participates in achievement of Organizational and Branch Key Result Areas (KRAs) related to Associate Engagement, Patient Experience and Health Outcomes.
