Sherman Hospital, a Magnet recognized hospital located in Elgin, Ill., is a 255-bed facility that features private patient rooms, a 15-acre geothermal lake, an advanced Emergency Department,  a free-standing Cancer Care Center, a spacious Family Birthing Center, a Heart & Vascular Center and more. Sherman also provides convenient, quality healthcare services at Sherman outpatient centers in Algonquin, Elgin, and South Elgin.

Position Requirements:

  • BSN or BS in a health related field
  • Registered Nurse 
  • 5 years Previous experience in Utilization Management or Case Management or related clinical experience
  • Previous management experience preferred
  • Strong assertiveness skills
  • Leadership experience
  • Ability to work cooperatively with a multidisciplinary team
  • Strong interpersonal skills
  • Ability to facilitate cooperation and communication
  • Superior assessment skills
  • Flexibility and the ability to prioritize work load, especially during high volume periods
  • Basic computer skills: Word, familiarity with email

 

Purpose:
To facilitate, implement and monitor the coordination of patient plan of care within the acute care setting into the continuum. The manager of Care Management is responsible for the operation, implementation and evaluation of services and processes of the department and the collaborative relationships with physicians, nursing staff and other department integral to the plan of care. The manager creates an environment that supports and sustains effective communication with internal and external customers and is reflective of the MVP. Through these processes, the staff is supported and given tools to have a positive impact upon the patient and families that are served at Advocate Sherman Hospital promotes quality care and cost effective outcomes.

Accountabilities:

  • Ensures clinical care process development and facilitation.
  • Care coordination/discharge planning, and facilitation and evaluation of programs
  • Utilization review/status mangement and compliance.
  • Outcome management/Quality Management
  • Manages and implements the activities of the Care Management Department.
  • Facilitates professional growth and development of staff.
