Position Requirements:

  • RN license
  • Master's Degree in Nursing, Business Administration, Health Care Administration, or related field.
  • 10 years of experience in care management and/or managing financial risk populations.
  • Strong leadership, communication, mentorship and consensus-building skills to align competing agendas across inter-departmental/organizational departments to drive success .
  • Skilled at diplomacy, assertiveness and ability to tolerate ambiguity.
  • Knowledge of relevant technical aspects of risk-based contracts in order to manage health plan delegation. (i.e. Stars, coding).
  • Strong analytic, financial, quantitative and problem solving skills.
  • Understanding of care management priorities and models for cross-continuum performance against priorities.
  • Ability to work in a highly matrixed, management by influence type of organizational structure.
  • Demonstrated competency in large scale deployment of change initiatives.
  • Demonstrated ability to develop strategic and tactical plans to meet objectives in a large healthcare organization.
  • Experience with budgetary accountability and efficient management of resources and staffing.
  • Expert in managed care and population health with a commitment to continuous learning.
  • Commitment and ability to develop a culture of high reliability, excellent patient and caregiver experience as well as metric-driven performance. 
  • Position requires travel and will be exposed to weather and road conditions.


Purpose:
The VP Integrated Care Management (VP, ICM) is accountable for determining the strategic vision and priorities for care management programs and other solutions that directly align with Advocate Aurora Health Care's business imperative to drive outcomes that are consistent with the Triple Aim. This includes the integration of the care management infrastructure at the system level to ensure operations and work processes are efficient, innovative and support a shared structure of care coordination in a way that delivers results relevant to the quality and efficiency targets of the organization. In addition, this role is accountable for the oversight and execution of employer-based care management programs as well as Clinical Documentation Improvement (CDI) across Advocate Aurora.

 

Accountabilities:

  1. Directly responsible to deliver on the vision, strategies and performance of cross-continuum care management (inpatient, outpatient and ED) and it's contribution to value~based contract performance along with attention to hospital value-based purchasing and compliance needs.   2. Areas of oversight include population-stratified care management, risk~based utilization, referral and disease management solutions.
  3. Accountable for the management of the Clinical Documentation Improvement (CDI) Program across Advocate Aurora.
  4. Collaborates closely with post acute, behavioral health, nursing and enterprise pharmacy programs focused on managing risk populations.
  5. Provides an evidence based framework and direction for all areas of oversight, reliably implementing best practices to perform at top decile relative to regional and national benchmarks.
  6. Ensures services are coordinated and aligned with clinical leadership across the continuum, supporting the development and execution of policies and procedures / guidelines.
