Purpose:
Provides leadership and management for the Post~Acute Network Specialty programs, these programs may include but are not limited to Home Based Palliative Care, Physicians at Home program, and Transitional visit program and those programs identified that will support the population health initiatives of the organization. . Monitors and responsible for the quality and metrics of the identified programs. Develops collaborative relationships both vertically and horizontally with regional stakeholders, acute care hospitals, medical groups, home care, skilled nursing facilities, long term acute care, assisted living, and inpatient rehab. Has economic accountability to decrease MSPB and total cost of Works in collaboration with population health to develop and implement programs to decrease total cost of care. Continuously discovers and articulates business impact, provides insights and operationally supports the successful execution of goals and initiatives to transform the organization's position in the marketplace and enhance Advocate Aurora Health competitive position now and in the future. This position would have responsibility across both states, would be very visible across the system serving on key committees representing the Advocate Aurora Post Acute Network.


Position Requirements:

  • Master's Degree in Health Administration, or Master's Degree in Business Administration, or Master's Degree in Nursing, or Master's Degree in Allied Health Field
  • Minimum of 10 years of progressive health care leadership experience in driving operational and/or strategic programs.
  • Experience in a Post Acute setting is desired along with settings within a complex integrated health care system.
  • Experience in payor relationships and managing risk relationships preferred, community based managed care or case management within post acute or acute environment. Knowledge of post acute care delivery and various external market forces that affect the post acute network
  • Excellent communication and interpersonal skills to successfully interact across the enterprise.
  • Ability to sucessfully and positively interact with physicians and all levels throughout the organization.
  • Knowledge of acute and post-acute care regulatory guidelines and requirements
  • Ability to understand payor contracts and incentives



Accountabilities:
Develop and implement the Specialty programs within the Post Acute division. These programs may include but are not limited to Home Based Palliative Care, Physicians at Home Programs and Transitional visit program. Monitor business and marketing plans to stimulate growth and increase capture within the Advocate Aurora Post~Acute Network for
