Part Time .5 Day Shift


Position Requirements:
B.A./B.S or equivalent in counseling psychology from an accredited college or university a religious professional (clergy, religious or theologically trained lay person)who has a combination of documented formal training in bereavement counseling and supervised counseling experience. Total academic and supervised work must equal five years.
Knowledge of regulatory compliance for bereavement programs in hospice, experience in coordination of hospice bereavement programs or their equivalent.
Excellent communication and human relations skills public speaking skills knowledge of customs and rituals of individuals of diverse cultural, ethnic, racial, religious and economic backgrounds Knowledge and experience in ministering to persons who are dying and their grieving family and friends. Knowledge of risk assessments related to bereavement. Certification in Bereavement counseling or 2 years equivalent experience. Ability to travel extensively within AHHS service area. Rotates on call responsibilities for after hours and weekends. Required to make visits to private residences which may not be accessible to handicapped individuals.

If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.

The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties efficiently,, results oriented and customer focused manner. This position is a critical component of the operating unit and as such meeting individual performance expectations and contribution to team performance goals is required.

Provide assessment of bereavement needs and identify risk as well as provide coordinate ,spiritual and bereavement counseling to patients, families/significant others and staff. Acts as a liaison with/for local community resources, provide community education and linkage to advocate and community bereavement support groups.

Develops and manages the hospice bereavement program.
Provides support to associates experiencing grief.
Responsible for developing relationships between hospice, religious communities, funeral directors, professional and lay groups that provide resources for those experiencing loss.
Participates in the QA/CQI process for Advocate Hospice.
Practices MVP. Demonstrates Advocate Health Care's leadership competencies. Encourages and sets expectations for staff to do the same.
Contributes 'best thinking” to the organization, share experiences, education and insight with peers, associates and home care leadership. Question/challenge the status~quo and suggest better ways of doing things. Provides leadership related to bereavement needs at the team meeting.
Practices and creates an environment that supports and sustains effective communication with internal and external customers. Develops and sustains relationships that enhances effective coordination of care and increases referral base.
