Advanced Practice Registered Nurse (APRN)-Heart Failure Clinic

 

Advocate Illinois Masonic Medical Center, Chicago (Lakeview area)

  • Full time; 80 hours a pay period

JOB and UNIT SUMMARY:

The Heart Failure Clinic proactively manages patient’s heart disease and improves their quality of life. The clinic reinforces prescribed medication and treatment plans for each patient. The clinic’s goal is to break the cycle of repeated hospitalizations.

 

The APRN will be responsible for obtaining patient’s medical history/medications/H&P, assessing for risk factors, recommending further testing, standardizing practice and patient outcomes. APRN will supervise the Heart Failure Infusion Clinic and provide education for this very complex patient population. Mostly Outpatient but may have some Inpatient accountabilities. Population in this clinic expands beyond heart failure to pulmonary hypertension, LVAD shared clinic etc.

 

  • Completes and documents accurate comprehensive, episodic history and physical assessments
  • Performs ordering, conducting, supervising, and interpreting diagnostic laboratory and imaging studies
  • Prescribes treatments, and therapies not limited to medications, diet, supplies, prosthetic devices, and equipment
  • Implements primary and secondary prevention strategies and management of acute and chronic health problems
  • Formulates a comprehensive plan of care in collaborating with the patient and interdisciplinary team
  • Advocates for patients, families, co-workers, the community, and the nursing profession
  • Provides feedback to and consults with leadership in assessment, problem-solving, decision-making, and evaluating system issues and models of care

 

The APRN-Certified Nurse Practitioner (CNP) reflects the mission, vision, and values of Advocate Health Care and adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and other regulatory and accreditation standards. The APRN-CNP works independently and collaboratively as part of the interdisciplinary team to provide health promotion, disease prevention, health education and counseling as well as the diagnosis and management of acute and chronic diseases through high-quality, cost-effective delivery of care focused on population health. This includes the assessment, diagnosis, and management of patient problems by prescribing pharmacologic and non-pharmacologic treatments. The APRN-CNP is licensed, board certified, credentialed and/or privileged as an independent practitioner to practice at the full extent of their licensure and within their defined scope of practice. The primary goal of the APRN-CNP is to function in a clinical role to provide comprehensive population based health care services and may serve as a primary care provider and/or consultant for individuals, families and communities in a variety of settings.

 

REQUIRED QUALIFICATIONS:

  • Masters of Nursing Degree as APRN-CNP
  • 2 years RN experience in cardiac nursing is preferred
  • 1 year APRN-CNP experience in cardiac setting preferred; Heart Failure clinic and LVAD experience a plus.
  • National certification as a CNP from an accredited body
  • Current Illinois APRN license
  • Current Illinois Registered Nurse license.
  • Current Illinois Controlled Substance License within 6 months of hire
  • Eligible for DEA registration; Possession of a current DEA Registration within six months of hire and may vary based on practice site
  • Current BLS Certification
  • Current ACLS Certification preferred
  • Well-developed interpersonal and communication skills.
  • Proficiency with an Electronic Medical Record
  • Travel may be required
  • Ability to work under stressful conditions and in difficult situations.
  • Able to respond quickly to patient needs and work at a fast pace -Routinely works with potentially infectious specimens and patients.
  • Work environment includes probability of exposure to adverse, hazardous or unpleasant conditions while caring for the sick
  • This position can recognize needs and behaviors of a variety of patient age groups; i.e., understand child growth and development, normal patterns of adolescent behavior and looks for signs of normal aging
  • Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
