LPN HOSPICE  Continuous Care Program

Territory: Chicagoland and Collar Counties--this team member will have to travel north city and north suburbs frequently

Shift: 7:00 p.m. till 7:30 a.m.

   

JOB and UNIT SUMMARY 

The purpose of this position is to help achieve the vision of Advocate Home Hospice by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required. 

The Licensed Practical Nurse (LPN) is responsible to the Clinical Manager for instructions on case management and accountable to the Clinical Manager for administration of patient care. The LPN will carry out selected aspects of the patient care activities to a group of patients in a manner that is consistent with the philosophy of Advocate Hospice. 

  • Assuring/Improving Quality of Care: Facilitates the patient and family's right to receive quality cost-effective care following Advocates MVP of Stewardship. 

  • Care and Service Coordination: Practices as an effective team member of the patient care team to formulate an integrated approach to care following Advocates MVP of Partnership.   

  • Leadership/Collaboration/Professional Development: Demonstrates respect and understanding for peers and other clinical disciplines and participates as an effective team member of the patient care team to formulate an integrated approach to care following the Advocate MVP of Excellence.   

  • Patient Relationship: Develops and maintains a therapeutic relationship throughout the healthcare continuum following Advocates MVP of Compassion   

  • Business Development and Growth: Demonstrates behaviors that enhance productivity and an understanding of financial and economic conditions in the health care environment.   

  • Patient Education: Demonstrates a commitment to meet the learning needs of patients and their families. Application of a systematic process involving optimal level of self-care for patient and optimal quality of life of patient family and significant other under the supervision of the RN following Advocates MVP of Equality. 

 

REQUIRED QUALIFICATIONS 

  • Current LPN licensure in State of Illinois 

  • 2 years of Medical/Surgical clinical experience 

  • CPR certified Basic knowledge of computer use (i.e. Microsoft Word, email access and use).
