Position Requirements:
MSW from accredited school of Social Work and field experience within a hospital setting.
The candidate must recognize age specific needs and behaviors.
Must adhere to NASW code of ethics and standards.
Demonstrates postgraduate competence in counseling and discharge planning with an intership n a hospital setting. .LCSW (Licensed Clinical Social Worker), or
LSW (Licensed Social Worker), or
ACSW (Academy of Certified Social Worker)

**Zero Hour Position**

Pick up as needed, weekend and holidays (coverage of leaves, vacation, etc)

Purpose:
To impact both the efficiency and the effectiveness of the delivery of patient care by providing a care management system which promotes quality care and cost effective outcomes. The SW Care Manager serves as a liaison for the physician and multidisciplinary team members in coordinating the patient's plan of care within the continuum of care. As a member of the health care team, the SW Care Manager promotes the integration of resources and services in the most efficient and effective manner to achieve patient outcomes. The activities of the SW Care Manager include identifying high risk admissions, performing comprehensive assessment, collaborating with the physician to manage the patient's stay in the hospital, monitoring the patient's response to
treatment/hospitalization, and facilitating the patient's return to the community with appropriate assistance and support.
