LPN HOME HEALTH 

  

Territory: Bloomington, IL 

Shift: Day    

Position: Registry

LPN with Pharmacology certificate preferred but not required

Must be able to work 1-2 days per week 

 Must be able to work 1weekend/mo. and 1 summer and 1 winter holiday

 

    

JOB and UNIT SUMMARY:   

 

The Licensed Practical Nurse (LPN) is responsible to the Clinical Manager for instructions on case management and accountable to the Clinical Manager for administration of patient care. The LPN will carry out selected aspects of the patient care activities to a group of patients in a manner that is consistent with the philosophy of Advocate Home Health.  

 

Facilitates the patient and family's right to receive quality cost-effective care following Advocates MVP of Stewardship.  

Practices as an effective team member of the patient care team to formulate an integrated approach to care following Advocates MVP of Partnership.    

Demonstrates respect and understanding for peers and other clinical disciplines and participates as an effective team member of the patient care team to formulate an integrated approach to care following the Advocate MVP of Excellence.    

Develops and maintains a therapeutic relationship throughout the healthcare continuum following Advocates MVP of Compassion    

Demonstrates behaviors that enhance productivity and an understanding of financial and economic conditions in the health care environment.    

Demonstrates a commitment to meet the learning needs of patients and their families. Application of a systematic process involving optimal level of self-care for patient and optimal quality of life of patient family and significant other under the supervision of the RN following Advocates MVP of Equality.  

 

REQUIRED QUALIFICATIONS:   

Current LPN licensure in State of Illinois  

2 years of Medical/Surgical clinical experience  

Certificate in Pharmacology prefered 

CPR certified Basic knowledge of computer use
