Position Requirements:
Graduate from an accredited school of nursing
4 years as RN~1 year in home care/hospice
Excellent communication skills
Computer experience preferred
Excellent knowledge of Medicare, Medicaid and Third Party Reimbursement
Working knowledge of managed care environment
Customer service skills
Ability to work under competing demands
Critical thinking skills
Current RN licensure in Illinois
CPR certified May be required to be 'on~call”

If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.

The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required.

To provide a smooth transition from the acute care and skilled nursing facilities to Advocate Home Health Services' entities for those patients requiring Home Health/Hospice care. The Liaison works in conjunction with discharge planning staff to assist the patient/family in the initiation of home care services. Completes final assessment on complex referrals involving infusion therapies, complex wounds and/or enterals.

Promote continuity of care by facilitating appropriate and timely referrals of designated patients to Advocate Home Health or Hospice. Upon referral to Advocate:
Continually looks for ways to effectively use time and resources to deliver quality outcomes. Identifies ways to improve policies and processes by utilizing problem solving skills and PI tools.
Promote collaboration and coordination between the hospital and Home Health Company to achieve continuity of care and utilize health care resources more effectively. Coordinator recognizes the needs and behaviors of various age groups.
Responsible for keeping Manager informed of status at assigned facilities.
