Position Requirements:
Bachelor of Science in Nursing, Human Nutrition or related field.
3~5 years experience in clinical specialty.
Effective 1/1/2015: BSN preferred, AND with agreement to enroll in BSN completion program within two years of hire date and to be completed within four years of hire date. Graduate from an accredited school of nursing. Strong leadership skills.

Ability to communicate and work cohesively with diverse groups.

Excellent organizational and planning skills.

Excellent Presentation skills.

Specialty certification in Diabetes (CDE) required.
Current license in the State of Illinois as a registered nurse required.
CPR Ability to travel locally.
Ability to function in a fast paced environment with established time constraints.
Ability to handle multiple tasks and complete work within a short time frame.
Resilient and flexible in a changing environment.
Ability to cooperate and work well with others.
Ability to make sound and timely decisions.
Ability to flex hours in order to accommodate needs of patients.

If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.

The diabetes educator has the responsibility to support and ensure the implementation of education for the diabetic patient mainly in the out~patient setting. Functions as a resource for diabetes management. Provides leadership and promotes development of the patient care team for the education phase initiated for the in~patients and out~patients.
Uses appropriate theories of learning and behavior change in the development and implementation of diabetes self~management education. Oversees the planning, implementation and evaluation of the Diabetes Self Management Education program and provides intermittent updates and an annual review of the DSME program plan, teaching methods, content, resources available and program effectiveness. Provides consultation on clinical and diabetes standards of practice to nursing service areas and throughout BRMC. Complies with all standards set forth by state, federal and local law and other regulatory agencies. Is held accountable for compliance with all BroMenn policies and procedures, particularly those dealing with discriminatory acts. Displays the philosophy and values of the organization through daily behavior. Puts the mission statement into practice.

Responsible for the coordination of education of patients, significant others and community related to diabetes self management as part of the continuum of care
Serve as a resource in diabetes management for the healthcare team.
Incorporate activities as appropriate to ensure positive patient outcomes.
Initiate a program that will meet the National Diabetes Advisory Board (NDAB) Standards in order to obtain the Education Recognition Program (ERP) certificate from the American Diabetes Association.
Maintain and provide staff development for a core of patient education representatives whose function is to assist with unit patient education, materials/program assessment, planning, and development, evaluation and revision.
