LPN- Home Health

  • Must be able to work 1 summer and 1 winter holiday and rotate weekends
  • Must be able to travel south city and south suburbs

How You Will Make A Difference

  • Responsible to the Clinical Manager for instructions on case management and accountable to administration of patient care.
  • Will carry out selected aspects of the patient care activities to a group of patients in a manner consistent with the philosophy of Advocate Health at Home.
  • Assure and improve quality of care for patients.
  • Facilitate patient and family rights to receive quality cost effective care.

Position Requirements

  • License in the State of Illinois
  • 2 years of med/surg clinical experience
  • Ability to learn and use a computer based scheduling and documentation system.
  • Ability to communicate professionally both verbally and through written reports.
  • Good interpersonal skills.
