Day shift with some rotating holidays. 

 

Position Requirements:
Registered Nurse
3 years combined pediatric care and adult med./surg. experience
2 years infusion or pediatrics homecare experience as a RN.
Some introduction to intake coordination or care coordination with a homecare company.
Exceptional interpersonal skills~~able to work well with diverse personality types in a fast paced environment ~~internal and external customers.
Good knowledge of home infusion supplies, VAD, medications and equipment.
Strong knowledge of home pediatric services.
Good working knowledge of home care insurance benefits ~Medicare (Part A and Part B), Medicaid, Managed Care

Illinois RN License

Must participate in Manager 'on call� rotation.
Must be willing to operate personal vehicle to conduct occasional patient visits.
Position ~9:30A ~ 6:00P M~F, or 4 ~ 10 hour shifts each week.


If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.

Purpose:
The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Home Infusion and Pediatrics Service Teams and as such meeting individual performance expectations and contribution to team performance goals is required.

To coordinate nursing care with other interdepartmental services required for infusion and pediatric care at patients' residences. Nursing services are coordinated with pharmacy, delivery, and therapy vendors, as well as the homecare patient. This position also serves as the communication point for the patient care team with other homecare functions conducting referral, patient qualification and billing activities such as homecare liaisons, reimbursement staff and customer relations staff. It may also serve as a communication point with external referral sources.
