4 years as RN-1 year in home care

Job Description:
The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required. To provide a smooth transition from the acute care and skilled nursing facilities to Advocate Home Health Services entities for those patients requiring home care. The Home Care Coordinator works in conjunction with discharge planning staff to assist the patient/family in the initiation of home care services.
Accountabilities:
1. Promote continuity of care by facilitating appropriate and timely referrals of designated patients to Advocate Home Care. Upon referral to Advocate:
2. Promote collaboration and coordination between the hospital and Home Health Company to achieve continuity of care and utilize health care resources more effectively. Coordinator recognizes the needs and behaviors of various age groups.
3. Responsible for keeping Manager informed of status at assigned facilities.
4. Associate will work day shift but must be available to stay and complete hospital referrals as needed.
5. Must be available most Thursdays & Fridays and be flexible with other days of the week.
6. Must be available to work day before and after at least one summer and winter holiday.
Occasion weekends will be necessary.
This associate must be able to work at Good Samaritan in Downers Grove, South Suburban in Hazel Crest, Christ in Oak Lawn, and IL Masonic in Chicago.
