Job Description:

The purpose of this position is to help achieve the Vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the operating unit and as such meeting individual performance expectations and contribution to team performance goals is required. Provides managerial direction and operational expertise to ensure delivery of quality, cost effective and timely services to patients and families. Ensures and maintains patient care delivery in compliance with all local, state and federal regulations, accreditation standards and department objectives. Contributes to the strategic direction of Hospice by providing planning and development of staff and patient care initiatives. Assures adequate exchange of information and communication among associates. Coordinates the implementation of the interdisciplinary plan of care.
Accountabilities:
1. Management/coordination of clinical and therapeutic hospice services to assure appropriate quality care and timely reimbursement from third party payors.
2. Provides expertise to staff regarding clinical situations, quality of documentation, third party payor guidelines and policies and procedures.
3. Responsible for quality and timely clinical documentation.
4. Maintains costs within budget of cost center.
5. Responsible for achievement of agency, department and personal goals and objectives.
