Masters or Doctorate Degree in Speech Language Pathology. IL License Eligible.

Must be available at least one weekend per month and needs to cover one rotating holiday per year.

CPR certified within 30 days of hire.

Job Description:

To provide speech therapy evaluation and treatment at a multiple diagnosis level for patients that are referred by physician.
Accountabilities:
1. Evaluation and implementation of speech therapy treatment programs for patients with multiple diagnosis.Treatment procedures should be safely and effectively administered according to the department policy and procedures. Quality care should be delivered with positive outcomes.
2. Maintaining timely and accurate patient documentation. a. Evaluation of patient progress b. Accurate and timely discharge notes c. Consultation with physicians to keep them appraised of patient progress d. Patient billing and charges e. Patient attendance records
3. Maintain efficiency of treatment by coordinating activities with other Rehabilitation Service personnel (both professional and support personnel.
4. Other duties as assigned. If assigned to the Fitness Center, duties also include activities which support programming and other services available at that site.
5. Administers speech therapy evaluations/treatments. Confers with physicians regarding changes in and/or appropriate therapy programs for patients, prepares patient records in a timely manner.
6. Provides supervision to support personnel and speech therapy students.
