12 Hour shift 7pm-7am full time RN position in Long Term Acute Care setting
Good written and verbal communication skills Computer skills Technical skills in the field of specialty, including use of equipment typical on a patient care unit. Good time management and organizational skills Have knowledge or be willing to acquire knowledge about the current healthcare environment, i.e. managed care Clinical knowledge in area of expertise: Long term acute care preferred. Graduate from an accredited school of nursing Earn 10 continuing education credits annually Minimal 1 year of RN experience Has demonstrated proficiency for the requirements for NC I Current Illinois RN license Current CPR
Job Description:
Plans and sets goals which demonstrates a holistic approach to patients/families. Assumes leadership responsibilities at the unit level. Demonstrates a mastery of clinical skills in the LTACH setting.
Accountabilities:
1. Nurse/Patient Relationship: Develops and maintains a therapeutic relationship throughout the healthcare continuum 2. Assuring/Improving Quality Care: Facilitates the patient and family s right to receive quality, cost effective care 3. Patient/Staff Education: Demonstrates a commitment to meeting the learning needs of patients and families. Utilizes appropriate resources to meet those needs and achieve positive patient outcomes. Assists in creating an environment that promotes educational growth opportunities for nursing peers and colleagues. 4. Nursing Process: Demonstrates the use of nursing process as a problem-solving model. 5. Leadership/Collaboration/Professional Development: Demonstrates respect and understanding for peers and other clinical disciplines and participates as an effective member of the patient care team to formulate an integrated approach to care. Demonstrates leadership behaviors and is committed to the development of self and others. Nurse/Patient Relationships: Develops and maintains a therapeutic relationship throughout the healthcare continuum. Performance Criteria: 1. Demonstrates follow-up and follow through with all referrals initiated during hospital stay. 2. Has thorough knowledge of Advance Directives, Pain Assessment Protocol, Fall Protocol and Informed Consent. 3. Explains all test and procedures to patient prior to implementation. 4. Ensures utilization of customer service recovery process to resolve customer service conflicts 5. Demonstrates charge nurse performance skills 6. Coaches associates in responding to internal and external customers in a courteous manner. 7. Acts as a resource to other members of the healthcare team for the utilization of age appropriate written, audio, video, and verbal communication 8. Acts as a resource for the utilization of communication services to the developmentally delayed, vision and hearing- impaired
Assuring/Improving Quality Care: Facilitates the patient and family s right to receive quality, cost effective care. Performance Criteria: 1. Demonstrates ability to function in all roles associated with code blue team and can responds to patient care emergencies according to established protocols. 2. Demonstrates ability to recognize changes in patients condition and initiate necessary interventions. 3. Demonstrates knowledge of unit s process improvement activities and assures that the practice indicators are met, monitors and complies with process improvement activities and participates in unit process improvement planning and activities upon request. 4. Participates in unit and hospital wide DIG s and offers suggestions for process improvement. 5. Can accurately complete unit quality controls for point of care testing equipment. 6. Documents patient information by utilizing the appropriate forms, adding procedural notes as needed, updating the written care plan and assuring that vital patient data is recorded in the medical record. 7. Demonstrates flexibility and resiliency in meeting the workflow needs as patient acuity and activity changes by prioritizing care without assistance. 8. Demonstrates problem solving ability in conflict and crisis situations. 9. Demonstrates support for established unit practices and procedures
