BSN, or BA in a related healthcare field
6 years relevant nursing experience OR 2 years Case Management or Utilization Management experience with 4 years of nursing experience Current nursing license in the State of Illinois CPR certification Telemetry; ICU or ER experience or Care Manager Adult Hospital experience

Job Description:
To facilitate the timely coordination of quality healthcare services that meet an individual s specific healthcare needs in a cost effective manner, through communication, coordination, and collaboration. To work collaboratively with Physicians and other members of the multidisciplinary team, both internal and external to the organization by coordinating and supporting the appropriate utlization of resources and the transition to the most appropriate level of care along the continuum for the individual patient. Accountabilities: 1. Assists in the development, implementation, and evaluation of a multidisciplinary individualized patient plan of care that includes an appropriate discharge plan. 2. Coordinates patient activities ensuring efficient utilization of multidisciplinary resources. 3. Identifies the variances from established plans of care, pathways, algorithms and guidelines and facilitates the development of action plans. 4. Participates in identification of learning needs. 5. Responsible for professional growth Additional minimum qualifications: Specialty experience may be required in the following areas: Intensive Care, Cardiac, Medical, Surgical, Women's Health, Pediatrics, Emergency Room, Critical Decision Unit.
