Good Samaritan Hospital has a new opportunity for a Clinical Pharmacist Specialist with anticoagulation services expertise to join our team in providing anticoagulation management care for inpatients related to DVT Prophylaxis Assessment & Treatment Intervention, as well as, Anticoagulation Monitoring & Treatment Intervention.
The clinical pharmacist specialist is a clinician expert in their specialty area of practice who is responsible for providing clinical pharmacy services including the monitoring of patient medication therapy and acting as a resource for the provision of medication information and education for patients, physicians, nurses, and other allied health professionals. The clinical pharmacist specialist identifies, resolves, and prevents medication related problems through the intervention with physicians. Pharmacists supervise and direct pharmacy support personnel to ensure the timeliness of medication availability.
Accountabilities: 1. DEVELOPS, IMPLEMENTS, MONITORS AND PROVIDES EDUCATION FOR MEDICATION THERAPIES, POLICIES AND PROTOCOLS TO ENSURE APPROPRIATE MEDICATION UTILIZATION TO OPTIMIZE POSITIVE PATIENT OUTCOMES. 2. ENSURES SAFE, APPROPRIATE, & COST EFFECTIVE MEDICATION THERAPY FOR PATIENTS ACCORDING TO ESTABLISHED POLICIES, PROCEDURES, AND PROTOCOLS. 3. OVERSEES THE PREPARATION AND DISPENSING OF MEDICATION PER PHYSICIAN ORDER ACCORDING TO ESTABLISHED POLICIES, PROCEDURES, PROTOCOLS, STANDARDS, AND / OR REGULATORY AGENCIES. 4. CONTRIBUTES TO THE QUALITY AND EFFECTIVE OPERATION OF THE DEPARTMENT. 5. MAINTAINS COMPETENCE REQUIRED FOR CURRENT JOB TITLE / POSITION.
