Graduation from an accredited O.T. program. License to practice O.T. in Illinois or eligibility. CPR certified within
30 days of hire.

Must be able to work 1 weekend per month and have availability to cover some week days.

Job Description:

To provide Occupational Therapy services for patients/clients.with cognitive impairment, physical disabilities or developmental challenges and limitations. These services include assessment, treatment, program planning, implementation, related documentation and communication.
Accountabilities:
1. Performance of evaluations and treatment in a thorough and timely manner in accordance with established departmental and professional standards and procedures resulting in clinically accurate Occupational Therapy plan of care. .
2. Documentation follows guidelines set forth by JCAHO, AOTA, Managed Care and Rehab Services Policy and Procedure.
3. Maintain efficiency of treatment by coordinating activities with other Rehabilitation Service personnel, nursing, medical staff, and other ancillary departments.
4. Other duties as assigned. If assigned to the Fitness Center, duties also include activities which support programming and other services available at that site.
