RN BSN or equivalent experience. 3 years medical/surgical experience 2 years experience in a Medicare licensed home health care agency 1 year supervisory experience Current licensure in State of Illinois CPR certified. Excellent organizational, communication, and inter-personal skills. Familiarity with computer use.
Job Description:
The purpose of this position is to help achieve the Vision of Advocate Home Health Services by performing the duties of this position in a cost effective~ results oriented and customer focused manner. This position is a critical component of the operating unit and as such meeting individual performance expectations and contribution to team performance goals is required. Provides managerial direction to ensure delivery of quality~ cost effective and timely services to patients and families. Ensures and maintains patient care delivery in compliance with all local~ state and federal regulations~ accreditation standards and department objectives. Contributes to the strategic direction of Home Care by providing planning and development of staff and patient care initiatives. Assures adequate exchange of information and communication among associates. Coordinates the implementation of the interdisciplinary plan of care. Accountabilities: 1. ONGOING: Practices MVP. Demonstrates Advocate Health Care's leadership competencies. Encourages and sets expectations for staff to do the same. 2. ONGOING: Contributes best thinking" to the organization, share experiences, education and insight with peers, associates and home care leadership. Question/challenge the status quo and suggest better way of doing things. 3. ONGOING: Practices and creates an environment that supports and sustains effective communication with internal and external customers. Develops and sustains relationships that enhances effective coordination of care and increases referral base. 4. Staff and Patient Care Management 5. Reimbursement and Fiscal Management 6. Responsible for achievement of agency, department and personal goals and objectives 7. Other duties as assigned including Manager on Call rotation and patient visits as needed.
