Job Description:
To coordinate and manager the care of all patients as they move along the continuum of care to make sure that the patient received quality care and the hospital stays in a position to be fully reimbursed for all services provided. Assist with the education of the interdisciplinary team as it relate to the business side of healthcare. And to collect data concurrently to provide senior administration with reports that will assist with operational decisions. Accountabilities: 1. Demonstrates Job Knowledge: Assess all patients for medical necessity and level of care as they move along the continuum of care. 2. Demonstration of advanced level knowledge in Care Management. 3.
